The Embassy provides the following consular services: passport issuance to first-time applicants; passport renewals/ extensions/ amendments; issuance of travel documents; visa issuance; registration of births, marriages, and deaths of Filipino nationals; solemnization of marriages between Filipino nationals; legalization of documents, including authentication, notarials, and certification; taking of depositions;  service of summons; and processing of petitions for change of first name and correction of clerical errors.


 

CORE REQUIREMENTS FOR ADULT RENEWAL APPLICATIONS

  1. Personal appearance
  2. Duly accomplished application form
  3. Original passport
  4. Photocopy of data page of old passport
  5. Photocopy of latest residence permit

  1. Damaged / Mutilated Passport

  1. Change of Status / Last Name from Single to Married
    • Original PSA Marriage Certificate (if married in the Philippines) / PSA Report of Marriage (if married abroad)
  1. Change of Status / Last Name from Married to Single
    • Original PSA Birth Certificate
    • Original PSA Annotated Marriage Certificate
    • If annotated Marriage Certificate is not yet available, Apostillized Court Judgment / Decision and Certificate of Finality
    • Original PSA Death Certificate of spouse, for widow
  1. Dual Citizen
    1. If the applicant possesses dual citizenship from birth (i.e. one parent is a Filipino while the other parent is a citizen from which the applicant's other citizenship is derived; or, have both Filipino parents but was born in a country which applies jus soli citizenship; etc.)
      • Original and photocopy PSA-authenticated Birth Certificate/Report of Birth
    2. If the Filipino citizenship was reacquired or retained as specified in RA 9225
      • Original and photocopy of Identification Certificate issued by a Philippine Foreign Service Post (FSP) or by the Bureau of Immigration (BI)
      • Government Issued IDs (Refer to the list of valid Philippine Government Issued IDs or secondary citizenship/residence counterparts)
  1. Naturalized Citizen
    • Identification Certificate of Naturalization from Bureau of Immigration
  1. Filipino Citizen by Election
    • Affidavit of Election of Philippine Citizenship
    • Identification Certificate of Election from BI

 

REPLACEMENT OF LOST PASSPORT

  1. Personal appearance
  2. Duly accomplished application form

  4. Original PSA Birth Certificate (if photocopy of lost passport is not available)
  5. Original PSA Marriage Certificate (if married in the Philippines) / PSA Report of Marriage (if married abroad), for married women

VISA                          

VISA APPLICATIONS  (Download Application for Non-Immigrant Visa P1-2 Form)

  8. Hotel booking or Invitation letter from the Philippine sponsor, with copy of sponsor’s passport or any Identification card Employment certificate

 

Additional requirements may be requested on the following cases

  • For Non-Bahraini national: Residence Permit or re-entry visa for more than six (6) months at the time of travel
  • For recruitment purpose: Letter of Authority from the Philippine Overseas Employment Administration (POEA) authorizing the applicant to recruit/interview Filipino workers. Must apply for a Special Work Permit (SPW) at the Bureau of Immigration upon arrival in Manila
  • For Medical treatment: Medical Certificate attested by the Bahrain Ministry of Health and letter of acceptance from Physician/Medical Institute in the Philippines
  • If married to a Filipino citizen: photocopy of PSA Marriage Contract and copy of spouse’s passport
  • For Foreign Nationals of certain countries: List of Countries whose nationals require a Bahrain CID Good Conduct. (Click Here)
  • For multiple-entry visa application: letter of intent indicating the applicant’s purpose for multiple-entry to the Philippines

  6. No Objection letter from  employer/sponsor of each contracting party

  5. Affidavit of Delayed Registration of Birth (if not reported within one (1) year) (BD 10.340)

  4. Original Certificate of Legal Capacity to Contract Marriage issued by the Philippine Embassy Bahrain
  5. Original PSA Birth Certificate
  6. Original Certificate of No Record of Marriage (CENOMAR) issued by the Philippine Statistics Authority (PSA) issued within the last six (6) months and duly Apostillized by the Department of Foreign Affairs.
  7. Original Parental Consent if between the ages 18 and 21 yrs. old or Parental Advice if ages 22 and 25 yrs. old. If parents are in the Philippines, Parental Consent/Advice notarized by the nearest notary public, and authenticated by Regional Trial Court and Department of Foreign Affairs (DFA)
  8. Affidavit of Delayed Registration of Marriage (if not reported within one (1) year) (BD 10.340)

REPORT OF DEATH

  1. Duly accomplished Report of Death form (available at the Embassy and downloads section of the website)

  3. Photocopy of the passport of the deceased
  4. Embassy’s certification on the Shipment of Remains

 

 SHIPMENT OF REMAINS      

  1. Employer/representative or next-of-kin of the deceased should present to the Embassy the original passport of the deceased, the Death Certificate/Medical Notification of Cause of Death issued by the Bahrain Ministry of Health and No Objection Certificate (NOC) of the shipment of the remains issued by Bahrain Ministry of Foreign Affairs
  2. Submit the address and contact number of consignee in the Philippines (next-of-kin or funeral parlor), and a Letter of Acceptance of human remains from the NOK
  3. Embassy’s Certification relative to the deceased will be submitted to the Ministry of Interior’s Criminal Investigation Directorate, for the issuance of “Declaration of the Removal of the Dead Body”
  4. Submit flight details of the shipment of the remains to the ATN Office, as basis for notification of the NOKs in the Philippines, and for reporting to DFA and DOLE-POEA/OWWA.
