RELEASING OF PASSPORTS

1. Personal appearance of the applicant
2. Original old passport
3. Original official receipt
4. Affidavit of Loss (for lost receipt)

* For passports to be claimed by another person, only immediate family members (spouse, parents, siblings, adult children) are allowed to claim the passport on behalf of the applicant, with an authorization letter and valid ID of the representative.

TRAVEL DOCUMENT

1. Personal appearance of the applicant
2. Accomplished Travel Document form
3. Affidavit of Explanation (BD10.340) (CASH BASIS ONLY)
4. Original passport & 1 photocopy
5. Four (4) pcs passport size photo (taken within the last 6 months)
6. Copy of confirmed airline ticket
